Chapter 131 How to Diagnose Abdominal Pain

A 30-year-old male patient, surnamed Liao.

Staff of a company in Guangdong.

Half a month ago, Mr. Liao started to have diarrhea, which became very thin and scattered. In addition, he had a bad appetite and didn't eat much. He barely ate a little and felt bloating and wanted to vomit. Nothing came in from above and tried hard to pull it from below. In less than half a month, Mr. Liao lost 3kg.

At first I thought it was because of the gastroenteritis caused by eating the wrong thing, so I bought some "beberine hydrochloride" in the pharmacy. After taking it for 2 days, I didn't have any effect. My friend said that my stomach caught a cold and I had to see a Chinese medicine doctor. For this reason, I also found a doctor in the Chinese medicine hospital. After taking Chinese medicine for one day, my abdominal distension became more serious the next day, so I had to go to the emergency department of the hospital.

When he arrived at the emergency room, Mr. Liao was very upset. He told the doctor that this diarrhea was not too serious, but it just affected his work and could not meet customers normally. He hoped to give some short and fast drugs to treat.

How many times can I take it a day? The doctor asked.

It's about 4-5 times, and I don't get much each time. But the most important thing is that I have a bad appetite, don't eat much, and I don't have the strength. Moreover, I have abdominal distension these days, and I feel full if I don't eat anything. Mr. Liao frowned and said, covering his stomach with both hands.

Does your stomach hurt? The doctor asked.

A little bit is not obvious. Sometimes it hurts a little. Mr. Liao said that it must be because I ate the oysters at the food stall before, which was not fresh enough and the garlic was added too little, which caused gastroenteritis.

There is no gastroenteritis that has been delayed for 2 weeks. The doctor said while prescribing the examination. If you have diarrhea for a day and a half, you still consider gastroenteritis. If it has been 2 weeks, you should be alert to other diseases.

You may need to be hospitalized for examination. The doctor does not raise his head and is preparing to issue a hospitalization form.

Doctor, I am not suitable for hospitalization for the time being, so I will give me some medicine first. Mr. Liao seemed to beg the doctor. He is too busy with work, so it takes ten days and half a month to stay in the hospital, and it cannot be consumed.

The doctor did not expect the patient to refuse to be hospitalized. After all, Mr. Liao refused to agree to the hospital for the time being. Then sign the patient and said that he did not agree to the hospital and prescribe some medication in the emergency room for the time being.

Then use some medicine according to gastroenteritis. If it doesn’t work in three or two days, you still have to go back to the hospital for examination. The doctor said with great thought.

And you need to do an electrocardiogram, which is routine. The emergency department doctor said.

I had an electrocardiogram, no problem.

Generally speaking, diarrhea lasts for 2 weeks, unlikely to be a heart problem, or a myocardial infarction, etc. Besides, the patient is only 30 years old, so it is really unlikely to be an electrocardiogram. However, the emergency department now basically does electrocardiogram routinely. Anyway, the price is cheap and there is no radiation, so I do it routinely.

It must be done. If you don’t, you have to sign it.

Do you want to draw blood? Take some blood and see. The doctor suggested.

I won’t smoke for the time being. I haven’t eaten these days and I’m not bleeding anymore. Mr. Liao said.

You will have problems when you see a doctor like this. If you don’t do this or that, the emergency department doctor is a little angry.

Anyway, you have explained everything you need to explain, just sign it. You can bear the consequences at your own risk.

In this way, Mr. Liao hung two bottles of potions in the hospital and went back to work in the company.

In the next few days, Mr. Liao ran around the hospital and the company.

It would be great if the potion was effective after being put down. The key was that it was not effective at all. What should be done in the past few days was still a bad appetite, and Mr. Liao felt that he was about to lose his whole body.

Three days later, I came to the emergency department again.

The doctor said firmly that your condition is not like ordinary gastroenteritis. You must take blood tests and be hospitalized for examination today, otherwise it may delay the condition. If you really don’t want to be hospitalized, go to the outpatient clinic of the gastroenterology department today to let them see what issues they are considering, and then treat them and don’t have to come to the emergency department again.

Then draw blood. It’s too difficult to register in the outpatient clinic. Mr. Liao said.

Do you have a fever? The doctor asked.

Haven't had a fever.

The doctor gave a doctor's order, took blood tests, and told Mr. Liao that B ultrasound should also be done. There is a high possibility of abdominal distension and diarrhea, and gastrointestinal diseases. B ultrasound is the most basic examination and must be done. If necessary, abdominal CT should be done.

Everyone agreed. Liao Xian's attitude towards the environment weakened. After all, it was still hard to get sick. I didn't want to be hospitalized before, because I was afraid of delaying my work. It's not easy to find a job these days, and I'm afraid I'll be dismissed again if I don't do it well. Moreover, the cost of hospitalization is definitely not low, and I don't know how much it can be reimbursed for medical insurance. But this time it's different. The treatment has been bad for a few days, and I'm afraid that it will cause big problems if it delays.

Mr. Liao is not a fool either, so of course he understands this principle.

The doctor examined Mr. Liao's stomach and found that the abdominal bloating was quite obvious, the intestinal rumbling sound was relatively weak, and there was slight tenderness, not significant. The liver area seemed to be a bit painful, and it was not clear whether it was a liver, gallbladder or other organs. There were many causes of abdominal bloating, so first of all, I would consider whether there was an intestinal obstruction? But Mr. Liao had farting and had diarrhea, which means that it was impossible to be an intestinal obstruction, even if it was not a complete intestinal obstruction. In addition, cholecystitis, pancreatitis, appendicitis and other surgical diseases will cause abdominal distension.

Be careful and be alert.

After finishing the b-ultrasound, I found the problem.

Fortunately, I did B-ultrasound, and fortunately, Mr. Liao no longer refuses this time.

B Super League saw that there were several hypoechoic areas (abnormal manifestations) in Mr. Liao's liver, the largest one was the size of a one-yuan coin. There were no abnormalities in the gallbladder, pancreas, spleen, etc., and there were no gallbladder stones, bile duct stones, etc.

What is the hypoechoic area of the liver? It is hard to say. It may be a liver abscess, which means there is an abscess in the liver, but it is not very similar. Why? Because ordinary patients with liver abscess will have obvious symptoms of fever and chills, and Mr. Liao has not had fever, and the pain in the liver is not very obvious, so it is not supported.

It may also be liver cysts, liver hemangiomas, etc. These two are benign diseases and generally do not have obvious symptoms. It is estimated that it is unlikely that Mr. Liao's symptoms such as diarrhea, abdominal distension, etc. will be caused.

So what will you see in this B-ultrasound? It is difficult to distinguish. The doctor recommends further abdominal CT examination, and an enhanced scan is required. Why? Because I am afraid that these hypoechoic lesions are liver cancer. Only with enhanced scans can we better judge whether it is liver cancer.

To further do abdominal CT, the emergency department doctor told Mr. Liao.

Mr. Liao gritted his teeth and agreed.

The blood draw result was also revealed at this time.

The blood test result also saw a significant incident. This abnormality made the emergency department doctor widen his eyes. Yes, today the emergency department doctor is still Lao Ma. Mr. Liao's blood routinely saw that the total number of white blood cells was as high as 30x10e9/l (reference value 3-9x10e/9). The total number of white blood cells is already quite high. Generally, only patients with severe infections will be so high. The patients themselves have blood system diseases.

Does he really have liver abscess? Doctor Gu Pei said. Teacher, can liver abscess explain his condition? B-ultrasound sees hypoechoic foci, high white blood cells, and diarrhea and abdominal distension. It seems that all are in line. The only thing that does not match is that he never has a fever, and it seems that the abdominal pain is not very obvious.
